My Free Passes only refill to 2 Passes every four hours, when I used to refill to 4 Passes.

New accounts will start with 2 Free Refill Passes every four hours.

After reading about twenty chapters, this will increase to 3 Free Refill Passes every four hours.

After reading about forty chapters, this will increase to the maximum 4 Free Refill Passes every four hours.

Free Passes do not stack. If you do not use your Free Refill Passes after 4 hours, you will not receive more Free Passes once your maximum Refill passes has been reached. 

If you previously had an Episode account that already unlocked the maximum 4 Free Refill Pass limit, and have now created a new account, you will need to unlock the Refill Limit once again by reading the required chapters.
